Special Olympics offers training and competition opportunities in 30 Olympic-type sports for athletes 8 years or older. For children with intellectual disabilities ages 2 through 7, Special Olympics provides a Young Athletes Program. Special Olympics coaches have a unique opportunity to work with athletes in competitive situations to assist in their training for life. As a grass-roots organization, Special Olympics relies on volunteers at all levels of the movement to ensure that every athlete is offered a quality sports training and competition experience. Individual donors, corporate partners and many others make it possible for Special Olympics to offer children and adults with intellectual disabilities the opportunity to develop physical fitness, demonstrate courage and experience joy through participation in the program.

The following forms are designed for the use of Clinical Directors when administering their individual programs:

Special Olympics Healthy Hearing Equipment Requests: If you are not able to acquire the necessary hearing screening equipment locally, or require additional units to augment what you have acquired locally, i.e., Bio-Logic otoacoustic emission (OAE) units and Biocoustics, Inc. tympanometers and audiometers, please complete the Healthy Hearing order form. Two versions of the form are available:

Complete either version of the form in its entirety; omitted information will delay fulfillment. Please review the detailed instructions and important additional information available.

STATUS CHECK: To learn the status of your equipment request, review the most recent (16 December 2008) requests/events tracking spreadsheet (Microsoft Excel; 78K).
